History

Total Longterm Care was established in 1991 as Colorado’s first PACE provider. PACE (Program of All-inclusive Care for the Elderly) is an innovative system of care designed to meet the needs of nursing-home eligible individuals. By combining medical care, community-based home and health-care services, and day programs, PACE providers help these individuals stay in their own homes and communities and out of nursing homes. There are PACE programs throughout the United States.

Today, Total Longterm Care is one of the nation’s-best PACE providers. It features five metro area Day/Health Centers and a network of transportation to ensure that all participants have an easy ride to and from their homes. The program continues to grow, but the goal of the program, to help each individual in a personal way, remains constant.
