Services

The participant/member in Total Longterm Care has an individualized care plan that is created by a number of team members who are all invested in providing quality care. The participant/member’s family is also included in this care team so that Total Longterm Care helps the family caregivers maintain their own quality of life.

Clinic Staff
Primary Physicians include doctors who are available at the Day/Health Centers and may be seen by the participant at no extra cost and without an appointment. Primary care also includes Nurse Practitioners and Registered Nurses who staff the clinics in the Day/Health Centers.

Specialty Physicians include doctors whose practices are designed to treat specific conditions or illnesses such as cardiologists for heart care, podiatrists for foot care, psychiatrists, surgeons and others, according to each participant’s needs.

Occupational Therapists
Rehab and Equipment involves occupational therapists who help participants maintain function in the tasks of daily living. These may include help with bathing or dressing, recreation therapists to help participants continue to enjoy familiar activities, and physical therapists who help participants regain or maintain strength and flexibility. When necessary, adaptive equipment such as grip bars in a shower, walkers or wheelchairs, wheelchair ramps, and special lighting will be prescribed and provided by Total Longterm Care.

Prescriptions and Over the Counter Medications
All necessary prescriptions and medicines are included. The Total Longterm Care list of covered medications is updated on the Web site regularly.

Transportation
Transportation is provided at no cost to help the participant come into his or her Day/Health Center for appointments, meals, activities, and socialization. When a specialist appointment is needed, a Total Longterm Care van will take the participant to and from the appointment. Van transportation to and from the participant’s home is included in the program. Ambulances in the event of an emergency are also included.

Hospitalization
In-patient Hospitalization in a semi-private room is included in the program.

Assisted Living/Nursing Home Care
There may be times when it is necessary for the participant to stay in a nursing home for a short-term acute illness or for respite care. The participant, his or her family, and the personal-care team together may decide that a participant’s health has declined so much that he or she will need to live in a nursing home. The team will then make their suggestion to the Nursing Home Review committee. That committee has to approve any permanent moves to a nursing home.

Outpatient Services
Outpatient Services such as lab work, X-rays, and day surgery are included.

Family/Spouse Services
Services such as caregiver education and support, caregiver support groups, and advice as needed are provided with the program.

Day/Health Center Services
Day/Health Center Services are provided to give the participant easy access to regular medical monitoring and clinical care. Some participants come into the Center just to socialize; others may come for rehabilitation. The Centers are open Monday through Friday during normal business hours. Saturday hours are available at select centers. Activities are designed to meet an individual’s ability. Centers offer classes on computer and e-mail usage, regular craft circles, games, and discussions of current events. The Centers are bright and cheery, designed for the unique needs of elderly individuals.

Other Medical Services
Professional Services such as dentistry/dentures, optometry/glasses, audiology/hearing aids, and podiatry are included in the program.

Home Care Services
Care in the home is a key part of the Total Longterm Care program and includes light housekeeping, chore services, skilled nursing care, meal delivery, and personal care such as bathing and dressing.

The Advantages of the Total Longterm Care program include complete, coordinated care for the individual, ongoing preventive care, regular monitoring by a care team, and caregiver support and respite, all geared to enhancing quality of life for the participant and family caregiver.
